How to Complete Your Application

 

1.Fill Out the PDF: One per family

Complete the form on your computer or phone. Only one application is needed per household—be sure to list all students attending the school on the same form.

 

2.Print, Sign & Date:Parent or guardian required.

Print out your completed form and ensure a parent or legal guardian signs and dates the document at the bottom.

 

3.Submit Your Application:Two ways to return.

Return your signed form in one of two ways:

  • By Email: Send a scanned copy or photo to [email protected]

  • In Person: Turn in the physical paper copy directly to the school office.
